The genome of coronavirus encodes the spike protein, the envelope protein, the
membrane protein, and the nucleocapsid protein.

The spike protein is the most important surface protein.
The main functions for the Spike protein is summarized below:
(1) The spike protein mediates receptor binding and membrane fusion.
(2) The spike protein defines the range of the hosts and specificity of the virus.
(3) The spike protein can be transmitted between different hosts through gene
recombination or mutation of the receptor binding domain (RBD), leading to a higher
mortality rate.
(4) The spike protein is the main component to bind with the neutralizing antibody.
(5) The spike protein is the key target for vaccine design.

The Structure of Spike Protein

The spike protein is a large type I transmembrane protein containing two subunits,
S1 and S2. S1 mainly contains a receptor binding domain (RBD), which is responsible
for recognising the cell surface receptor. S2 contains basic elements needed for the
membrane fusion.

It’s been reported that 2019-nCoV can infect the human respiratory epithelial cells
through interaction with the human ACE2 receptor.

The Nucleocapsid Protein

Nucleocapsid is an unit of viral structure, consisting of a capsid with the enclosed
nucleic acid; it is generally inside the cytoplasm. The nucleocapsid may correspond to
a naked core or be surrounded by a membranous envelope depending on the
virus.The nucleocapsid protein (N-protein) is the most abundant protein in
coronavirus. The N-protein is a highly immunogenic phosphoprotein, and well
conserved and is often used as a marker in diagnostic assays.

Principle:
The spike proteins of SARS-CoV-2 commonly bind to the human angiotensin converting
enzyme 2 (ACE2) protein as a host receptor through their S1B domain. Receptor
interaction is known to trigger irreversible conformational changes in coronavirus spike
proteins enabling membrane fusion.
The SARS-CoV-2 Inhibitor Screening ELISA measures the binding of the RBD of the Spike
S protein from SARS-CoV-2 to its human receptor ACE2. Thus, this assay allows
identification and characterization of the effect of different inhibitory molecules

Principle:
The kit employs a blocking ELISA technique which mimics the virus neutralization
process. SARS-CoV-2-neutralizing antibodies primarily target the trimeric spike (S)
glycoproteins on the viral surface that mediate entry into host cells. The S protein has
two functional subunits that mediate cell attachment (the S1 subunit, existing of four
core domains S1A through S1D) and fusion of the viral and cellular membrane (the

Tocilizumab, a monoclonal antibody against interleukin-6 (IL-6), has emerged as an alternative
treatment for COVID-19 patients with a risk of cytokine storms recently.
In these studies, clinicians have discussed the treatment response of Tocilizumab therapy in
COVID-19 infected patients. The demographic, treatment, laboratory parameters of C-reactive
protein (CRP) and IL-6 before and after Tocilizumab therapy and clinical outcome in COVID-19
patients were retrospectively assessed.
Tocilizumab appears to be an effective treatment option in COVID-19 patients with a risk of
cytokine storms. And for critically ill patients with elevated IL-6, the repeated dose of the
Tocilizumab is recommended.Genentech, maker of another IL-6 inhibitor, Tocilizumab (Actemra),
is working with the FDA to initiate a randomized, double-blind, placebo-controlled phase III clinical
trial in collaboration with BARDA to evaluate the safety and efficacy of Tocilizumab plus standard
of care in hospitalized adult patients with severe COVID-19 pneumonia compared to placebo plus
standard of care.
The primary and secondary endpoints of the study include clinical status, mortality, mechanical
ventilation, and ICU variables.An open label, non-controlled, non–peer reviewed study was
conducted in China in 21 patients with severe respiratory symptoms related to COVID-19. All had
a confirmatory diagnosis of SARS-CoV-2 infection.
